One of the main goals of licensed professional nurses in home health is to allow patients to stay independent as much as possible while getting the care they need. This will enable patients to remain safe, comfortable, and secure while under supervised health care.

Duties & Responsibilities

Duties & Responsibilities

Home Health Nurses’ duties and responsibilities include the following, but not limited to:

  • Performs physical assessments.
  • Assesses and customizes patient needs and develops a plan of care with physicians.
  • Coordinates and oversees care plans with patients, family, and support staff (nursing assistants).
  • Oversees patient case management.
  • Manage patient’s medication regimen in collaboration with doctors and other healthcare professional’s orders.
  • Provides wound care and ostomy care per doctors’ orders.
  • Educating patients and families on health issues, management, and home safety.
  • Prevention of re-hospitalization Management of heart attacks, strokes, cancers, and other diseases per doctors’ orders.

Physical Therapy

Our licensed physical therapists speak multiple languages and have been recognized by several federal and state audits for improving a patient’s ability to move and their range of motion following severe strokes, injuries, and surgeries. They work to help patients regain physical functions like walking. With their help, patients regain strength and independence.

Physical therapy often helps to decrease the duration and severity of disabilities that arise from chronic diseases such as heart failure and COPD and helps patients return to their desired daily activities; in-home physical therapy sessions help patients understand how increased physical activity and better lifestyle choices can lead to weight loss, lower blood pressure, and improved mental health. An exercise program may also decrease future cardiac events and improve mortality.

Conditions that may be a good fit for home-care physical therapy may include

  • Post-surgical conditions like a total hip or total knee replacement
  • Stroke recovery and rehabilitation once discharged from a facility.

Licensed occupational therapists help our patients to manage everyday home activities better and to promote independence in tasks such as personal care, proper eating techniques, safe cooking, dressing, grooming, personal hygiene, and use of assistive devices.

Conditions that may be a good fit for home-care physical therapy may include

  • Treats patients through the therapeutic use of everyday activities.
  • Educates patients on how to do tasks in a new way following a stroke or other conditions that affect movement.

Speech Therapy

Licensed professionals assist in recovering and improving specific conditions, such as stroke or degenerative disease.
