General Job Descriptions:

Registered Nurses (RNs):
- Provide care directly in a patient’s home, administer medications, and assist with treatment plans as directed by physician orders.
- Monitor and assess patients’ medical histories, record and monitor conditions, and educate them and their families on better health management.
- Coordinate and supervise the work of other healthcare personnel.
- Work together with healthcare teams to develop and utilize care plans.
Licensed Practical Nurses (LPNs):
- Provide basic nursing care in a patient’s home, including administering medications and injections per physician orders.
- Monitor vital signs and report any changes to healthcare professionals.
- Assist patients with their activities of daily living, hygiene, and wound care.
- Ensure patient well-being by working with the healthcare team and following physician orders as directed.
- Maintain accurate patient records and document all necessary patient information.
Physical Therapists (PTs):
- Evaluate the physical abilities, mobility, and strength of patients.
- Build specified treatment plans to reduce pain, improve movement, and restore function.
- Use therapeutic exercises, and assisting devices to help patients regain motion.
- Teach patients and their families about exercises and ways to improve managing conditions.
- Monitor progress and modify treatment plans as needed.
Occupational Therapists (OTs):
- Evaluate the patient’s functional limitations and abilities related to daily activities at home.
- Develop personalized treatment plans to improve quality of life and independence.
- Help patients improve their motor skills, cognitive capabilities, and coordination.Modify the patient's home environment to promote safety and accessibility.
- Work together with other healthcare professionals to coordinate comprehensive care.
Speech-Language Pathologists (SLPs):
- Evaluate and diagnose speech, swallowing, and communication disorders.
- Develop treatment plans for speech, language, and swallowing improvement.
- Give therapy sessions and teach about ways to improve swallowing/communication disorders.
- Monitor patient progress, collaborate with healthcare teams, and adjust treatment plans as needed.
Certified Nursing Assistants (CNAs):
- Provide basic care to patients, including but not limited to bathing, feeding, and aiding the mobility of patients.
- Assist with patient hygiene, toileting, and dressing.
- Monitor vital signs and report all changes to healthcare professionals.
- Observe patients, document findings, and report information to the healthcare team.
- Maintain a safe home environment and assist in transferring patients.
Home Health Aides (HHAs):
- Personally care for and assist patients with daily routines and activities in their homes.
- Provide companionship and emotional support to patients.
- Assist patients with medication reminders and meal prep.
- Assist with bathing, dressing, and mobility.
- Maintain a clean and safe home environment and perform light housekeeping.
- Report changes in patient conditions to the correct healthcare professional.
Bilingual Homecare Coordinators:
- Speak with patients, their caregivers, and healthcare providers in their preferred language.
- Manage homecare plans and coordinate services for patients.
- Develop personalized homecare plans after assessing patients’ needs.
- Arrange/schedule doctor appointments for patients.
- Follow-up with coordination, delivery of care services, and patient satisfaction.
Bilingual Homecare Outreach Representatives:
- Build relationships between the home care agency and the community.
- Promote homecare services and educate people on available options.
- Schedule and attend activities, like presentations, events for the community, and health fairs.
- Provide info, answer questions, and help the patient enrollment process.
- Communicate with people in their preferred language.
